Different submit pages based on a questions answer
Here is an example:
A COVID form that users fill out and when they submit, they would land on one of two pages (options below occur after a question or business rule criteria is met):
- If they fail the screening, the fail submit page would have a specific message with pertinent information on what to do after failing.
- If the pass the screening, the pass submit page would have a specific message with pertinent information on what to do after passing, or the ability to re-direct the user to a URL after passing the screening.
A competing cloud forms product has this option available.

Yes, frevvo is great solution to do exactly that! Please see this article, which includes a COVID health screening form template you can install yourself. This particular template is configured to show a different message with different instructions at the end of the form based on whether the screening passes or fails.
You can further customize what the user sees when they click Submit by using a templatized string in your Form Action. You can configure this to show different messages or direct your employee to different landing pages, i.e. your company portal if the screening passes, or to another page with special instructions if the screening fails.
