Error for v 5.3.4 submission SQL query

submissions?q=count&output.type=opencharts&from=6-21-2014&to=7-21-2014

This is what I see every time I try to check if a form has been submitted properly when checking submissions instead of a list of viewable submission data. I am using MySQL, please help with this issue.
