Spurious http 501 errors using https form Url

We are using Live Forms 4.1.5 and our users are getting an HTTP error 501 not implemented. If they use HTTP instead of HTTPs the form appears to submit fine. How should we troubleshoot this?

Logs show following:
INFO: 15:11:57,434 INFO Resource:56 - 501 Not implemented - OST https://test.com/frevvo/web/tn/FormCreation/user/FormDesigner/app/_ymJWsSgHEeGnMecfTifOpQ/form/_LeuCO9qBEeGJ5cbGyuA3lw

  Your  logs message show OST instead of POST. Something is eating the first character.
  
  Per our prior experience the problem is caused by HTTPS proxy/forwarder which handles the HTTPS request. Please verify the HTTPS proxy configuration to check if some setting in there is incorrect. This has solved the problem in the past.
