Google Connector is not working

I'm encountering two issues.

First, a form was submitting entries to a google sheet (as it was built to do), but when I double checked the connector, the document actions setting was set to "do nothing." 

Second, once I enter the google connector information and click save, it shows that it's blank and gives me an error that I need to fill in the fields.

Any advice would be great!
